Welcome to the IPC, Outbreaks and AMR Stewardship (IOS) Team’s situations log web tool
This tool should be used by UKHSA staff to capture information on relevant health and care associated, or antimicrobial resistance related, situations (e.g. incidents and outbreaks) that UKHSA are aware of or actively involved in. The data captured will be used to monitor ongoing situations, and record how they are being managed, and any lessons learned.
Information can be added for new situations using the ‘Create situation’ button, or information on existing situations can be updated using the ‘Edit’ button for that situation. Please do not enter Patient Identifiable Information (PII) anywhere in the form.
